  • Fax your request to: 408-347-7005
  • Please include:
    • Student's signed release/authorization form
    • Student's full name
    • Date of birth
    • Year of attendance and/or graduation

 

Diploma Pick-up:

  • Only at the Registrar's Office and during regular business hours. Photo ID required.

 

Withdrawal:

  • Parent/guardian must be present to complete the process 

  • Parent/guardian ID required

  • Books/items checked out to the student must be returned 

Change of Address

  • Complete the "Change of Address" Form (located under the Forms section on the top, left side of this page)
  • Provide three (3) acceptable mail communications, listed above.
  • Provide proof of residence (choose which one applies): 
    • Lease/Rental Agreement in Parent/Legal Guardians name
    • Notarized statement by the homeowner (Must use Co-Resider Form, located under the Forms section on the top, left side of this page)
    • Current Property Tax Bill or Escrow paperwork

NEW STUDENT REGISTRATION:

You and your student must live in the EVHS Attendance Area Boundaries and show proof of residence in order to pick up a registration packet.

"Option Area" attendance area boundaries: 

Mt Pleasant/Evergreen Valley & Silver Creek/Evergreen Valley Option Areas

Students who live in the Mt. Pleasant/Evergreen Valley or Silver Creek/Evergreen Valley assigned option areas will be automatically assigned to a school (Mt. Pleasant High School or Silver Creek High School) based on their residency.  For example, students residing in the Mt. Pleasant/Evergreen Valley option area, but whose residence is in the Mt. Pleasant High School boundary attendance area, will be automatically assigned to Mt. Pleasant High School for enrollment. Any student residing in an option area will have first priority to elect to attend Evergreen Valley High School during open enrollment, February 1 through June 15, for the 2019-2020 school year. Should enrollment at Evergreen Valley High School reach maximum enrollment capacity, students will be placed on a waitlist and, when enrollment falls below maximum capacity, will be assigned to Evergreen Valley based on waitlist order.

***Please note: Students coming into our school district from another country will need to take an English proficiency test to determine proper school placement. Current TB Risk Assessment results must be provided prior to scheduling the English proficiency test. 

 

PLEASE PROVIDE THE FOLLOWING DOCUMENTS:

  • Official Birth Certificate (government issued only, not from hospital); foreign certificates must be translated
  • Parent/Guardian’s Current California Driver’s License/Identification Card
  • Immunization Record. Must include the record of Tuberculosis Skin Test
  • Students enrolling from outside of Santa Clara County will be required to have their healthcare provider complete the Santa Clara County Public Health Department Risk Assessment for School Entry Form. TB Assessment form is located under the Forms section on the top, left side of this page. 
  • Verification of withdrawal from previous school
  • Transcript with 2nd Semester Grades/Credits
  • Conditional Documentation: Caregiver’s Affidavit (If Applicable)
  • Current IEP or 504 must be submitted at the time of registration (If Applicable)

CHOOSE WHICH APPLIES TO YOUR HOUSEHOLD AND SUBMIT THE APPROPRIATE DOCUMENTS: 

1. If parent/guardian owns the house, provide the following:

  • Current Property Tax Bill or Escrow paperwork 

                                                          AND

  • 3 Acceptable mailed communications (see below) 

2. If parent/guardian rents the entire house, provide the following:

  • Current Legal Lease/Rental Agreement in parent/guardians name

                                                                   AND 

  • 3 Acceptable mailed communications (see below) 

 

3. If parent/guardian rents a room or multiple rooms of the house, provide the following:

  • Lease/Rental Agreement in Parent/Legal Guardians name

                                                       -OR-

  • Notarized Statement by the home owner (Must use the form provided in the registration packet)

                                                                  AND

  • 3 Acceptable mailed communications (see below)
